I, Douglas E. Taylor dba Taylor Made Images, (Photographer) for good and valuable consideration, the receipt of which is acknowledged, give to _____________________________ (Model), his/her legal representatives, successors, and all persons or corporations acting with his/her permission the absolute and irrevocable right and permission, with respect to any and all photographic portraits or pictures taken by me, alone or included with others, and the negatives, prints or transparencies to:

To use, reuse, publish and re-publish the same in whole or in part, individually or in conjunction with other photographs, in any medium only for the limited purpose of illustration, promotion, advertising and trade.

To use my name in connection therewith if so desired.

I further release the Model, or others for whom he/she is acting, from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the proper commercial or artistic use of these images, including without limitation any claims for libel or invasion of privacy, unless it can be proven that said reproduction was maliciously caused, produced and published for the sole purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn and indignity.

I hereby warrant that I am of legal age to contract in my own name and am the legal copyright holder. I have read the above authorization, release and agreement, prior to it's execution, and am fully familiar with the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns. All other rights reserved.
